This form is for returning students; first time applicants download and fill out this form instead. You may either mail it, fax it, or scan and email it to us.

Rules and Requirements

Tuition Assistance - Rules and Requirements

You need to be eligible.
- Have worked at your facility for a year or more
- Work a minimum of 15 hours per week
- Work in a unionized position
You need to submit your application on time.
-Our deadlines differ depending on the program you're in.
* If you're doing multiple terms toward a degree/license/certification, you'll need to submit applications for EACH term, by the corresponding deadline.
* If you are attending an online course or one-term program, you will need to submit your application to us at least 21 days before that class starts.
You need to apply for all other available financial aid, including FAFSA.
You need to let us know how you're doing,
- We base our payments and correspondences on the information you provide. If this information changes in any way (drop/add course, change of contact info, leave workplace, etc) you are required to inform us of these changes.
If you're using a voucher, you need to send us your grades when you finish a class.
If you're using a reimbursement, you need to send us your grades and your bill from the school when you finish a class.
You need to pass your class to receive another voucher. If you do not pass, you may still use our funding but we will only be able to reimburse you until you've passed a class.
You need to continue working at your facility while you're in school, at least 15 hours per week.
-If your average drops, or you leave your job while on reimbursement, you will not receive a check from us when your class is finished.
-If your average drops or you leave your job while on voucher and we have already issued that voucher to the school, you will not be required to pay it back, but you will no longer be able to use Tuition Assistance until you are eligible again.
