You must maintain employment to be covered. If you take a Leave of Absence, are laid off, or receive a disciplinary suspension, your coverage may lapse.
To find out if your dependent is eligible, or for any other important eligibility requirements specific to your facility, please check your union contract or call the SEIU Healthcare PA Plan at 1-800-877-6490.
If your spouse or dependents change at any point, you must notify your human resources office within sixty (60) days. Other special events such as a change in employment status, work schedule, (or those of your spouse or other eligible dependents) may allow you to make additional changes to your coverage.
